How to get started

Start the process for services by clicking "Get Started Today" below. You will be required to input your insurance information, so please don't start the form until you have your insurance information with you. 

Upon completing our confidential contact form, you will get a confirmation email saying if it was successfully submitted. We will also receive an email that we got a contact form from you. We will then have your insurance information verified to determine your financial responsibility for services based on your insurance plan. If our office needs to reach you for any additional information or clarification, they will contact you at the phone number you provided.

Once your insurance information is verified and a counselor based on the preferences you selected is available, our office will contact you via the phone number you provided to get you scheduled for your intake appointment. When your appointment has been scheduled, you will receive an email to access your "Client Portal" and you will receive intake paperwork to complete with electronic signatures. Please complete this paperwork in full before your intake appointment. We look forward to working with you!
